Criteria 2

Inclusion Criteria

Does the patient meet ANY of the following criteria?	Hospitalized > 96 hours
	Body weight < 47 kg
	Unable to receive nourishment (oral, NG)
	Life expectancy < 8 weeks
	The patient does not meet any of the listed criteria

Does the patient meet ANY of the following criteria of high bleeding risk?	Significant GI, pulmonary, or GU bleeding, chronic peptic ulcer disease or gastritis
	Clinically significant bleeding within 6 months
	Any IC bleeding, cancer, trauma or lesions within 3 months
	Received fibrinolysis or thrombolysis in prior 30 days
	The patient does not meet any of the listed criteria
