Intraventricular conduction delay history and symptoms

Intraventricular conduction delays (IVCD) are abnormal ECG pattern, which may or may not be associated with symptoms. Complete history to find out underlying cardiac condition would be the best approach in asymptomatic patients.

The following history should be elicited while evaluating a case of intraventricular conduction delay :

• History of palpitations, decreased general activity, fatigue, reduced exercise tolerance, dizziness and syncope
• Family history of ventricular arrhythmias, sudden cardiac death, heart blocks and myocardial infarction
• History of cardiac surgery
• Past history of myocardial infarction, myocarditis, cardiomyopathy and hypertension
• History of congenital heart disease in case of children

Patients with isolated BBB without underlying heart diseases are usually asymptomatic. Such people do not require further diagnostic evaluation. In patients with underlying heart disease symptoms are usually due to the heart disease rather than IVCD’s itself. Such symptoms include:

• Palpitations
• Fatigue
• Presyncope
• Syncope
• Reduced exercise tolerance
