Surgery is not the first-line treatment option for patients with onychomycosis. Surgery is usually combined with medical therapy.

Nail surgery is not the first line treatment option for patients with onychomysosis as its painful and may be disfiguring. Surgery is usually combined with medical therapy to increase the efficacy. Indications for surgery include pain due to paronychia, patients not able to use systemic antifungal agents due to contraindications or the presence of drug-resistant pathogens. [1]

Surgical approaches to onychomycosis treatment can include nail avulsion by mechanical, chemical, or surgical methods. Chemical removal is done by using 40-50% urea compound , usually in patients with very thick nails.

Several laser devices have been used to treat onychomycosis, including Nd:YAG lasers and diode lasers.

Photodynamic therapy has been reported as effective in a few studies.
