Primary antibiotic prophylaxis against peritonitis is indicated in most intra-abdominal operations whether elective or acute exploration.

Primary Prevention
Primary prevention for secondary peritonitis with antibiotic prophylaxis is required in patients with any of the following conditions:[1][2]
- Patients undergoing elective gastrointestinal or genitourinary surgery, most commonly colonic surgery[3]
- Elderly patients with acute cholecystitis , dysfunctional gallbladder, obstructive jaundice , or choledocholithiasis and who are going for biliary surgery
- Recent history of abdominal trauma but without explorative laparotomy
