In TIMI Grade 4 thrombus, a very large thrombus with its largest dimension greater than or equal to 2 vessel diameters is present.[1]

Outlined in yellow in the image on the right is the thrombus lesion in the RCA; the thrombus is visually detected by a haziness in the color compared to the color of the adjacent artery. Note that the linear dimension of the outlined lesion is two times larger than the diameter of the adjacent artery
